The Oshkosh YMCA is committed to providing a safe, welcoming, and respectful environment for everyone. This

confidential form is available for staff, volunteers, members, and guests to report concerning behavior, misconduct, unsafe conditions, or policy violations occurring at our YMCA facilities or programs.

You may submit a report about behavior involving members, staff, volunteers, or guests. Examples include suspected child abuse or neglect, harassment, discrimination, safety hazards, or any actions that compromise the safety and well-being of others.

Reports are reviewed by authorized YMCA leadership and handled with care and discretion. You are welcome to submit your concern anonymously. If you choose to provide your contact information, we may follow up with you for additional details if necessary.

Please note: If this is an emergency or you believe someone is in immediate danger, please call 911.
